Multiple Pigmented Basal Cell Carcinomas of the Eyelids

A 65-year-old man with a history of various facial skin cancers presented with multiple, bilateral, pigmented eyelid lesions, including an ulcerated lesion on the left lower eyelid with pigmentation at the margin of the lesion. Biopsy specimens from the right medial canthus (A [arrowhead]) and from the left upper and lower eyelids (B [arrowheads]) were all diagnosed as pigmented nodular basal cell carcinomas. A microscopic examination demonstrates a heavily pigmented nodular basal cell carcinoma from the right upper eyelid (C [hematoxylin-eosin, original magnification ×200]). Bleached sections reveal atypical basophilic tumor cells (D [original magnification ×200]). Immunohistochemical staining with a red reaction product was used to distinguish the S-100 protein from inherent pigmentation in the lesion (E [original magnification ×200]); the tumor cells demonstrate positivity for S-100 (red).
